Athletic and Extracurricular Registration

The athletic/extracurricular registration is only necessary for students who plan on participating in MTHS sports and/or extracurricular activities (ex. band, chess, robotics, student council, etc.) during the upcoming year. Any student participating in a sport/club/activity is required to submit 4 forms PRIOR to participating in any meetings, practices, games or events. Please print the forms below and submit at on-site registration OR scan the completed documents and email to Mrs. Darci Ohl at dohl@mths.us.

  1. MTHS Drug & Alcohol Testing Consent Form
  2. Impact Testing Form (consent to be tested if a concussion has occurred)
  3. Sports Medicine Consent Form (ISHA consent form)
  4. MTHS Athletic Permit Card

Sport physicals MUST be submitted during onsite registration (July 257th – July 31st).

Incoming Freshmen – An interest questionnaire has been emailed to all incoming Freshmen students. If you did not receive a link to record your extracurricular interest areas, please contact the MTHS Main Office and ask for Mr. Jared Hart (Athletics) or Mr. Kevin Hodel (Clubs and Activities)


Even though much is uncertain, MTHS activity sponsors are enthusiastically planning for the upcoming 2020-2021 school year. As we continue to learn more from authorities, activity sponsors will reach out to interested students regarding activity registration, sign-ups and try-outs.

Boys Golf

In order to tryout, a current physical and 4 forms must be completed and turned into the office by August 7.  Here is a link to the school website with the information: https://www.mths.us/2020-2021-activities-sports/


As of now, tryouts are scheduled to be held on the following days / times:

Monday, August 10. 8:30am. 9 -holes will be played

Tuesday, August 11. 6:45am. varsity will play 18 holes, JV 9-holes

Wednesday, August 12. 330pm. 9-holes will be played


All players must provide their own equipment and will meet at Metamora Fields.

When trying out, cheerleaders denote an option for participating in football, basketball or both.  There is a lot of uncertainty surrounding fall sports.  Right now we don’t feel comfortable having our students and parents make a decision about football or basketball without better information.  We will not be scheduling in-person or virtual tryouts at this time.  IHSA has not given any direction on what fall sports (football in particular) will look like.  Because of this, cheer tryouts are postponed until more guidance is given by the IHSA.   If you have a student who is interested in trying out for cheerleading, please send their name and grade to Coach Doty (ldoty@mths.us) and she will reach out to them when we are able to schedule tryouts.

Chorale is currently conducting virtual auditions right now
*Current Chorale members simply need to fill out a form stating they plan to return
*If students that are not currently in an ensemble missed the postings, they are more than welcome to audition late. That information is located on the Choir Website (https://www.mthschoirs.org/chorale-auditions-2020-2021.html) – we may hold another set of auditions for specific voice parts in the fall, but that is still up in the air

Dance Team

The first step in auditioning for the MTHS Dance Team is to send in a qualifying video and the dance team application form NO LATER THAN Friday, July 24th 2020. NO EXCEPTIONS! Please wear a black top and black dance shorts on your video tape. The video must be shared via a file sharing service (i.e. Google Drive or DropBox). Clearly label your video with: full name, address, home and cell phone numbers and email address. Please email the link to Coach Devery McCoy (Can be multiple links- but label all). A confirmation email will be sent when all material has be received by Coach McCoy. Fall Play


  • Fall Play audition materials are currently available on the Fall Play Activities Page of the MTHS website.
  • Read the Audition packet to find out how to submit your audition.
  • Make sure to submit your Google Audition Form as well as SHARE your videos to Mrs. McCormick by midnight, May 21.

Madrigal & Court Singers
Madrigal & Court Singers auditions will be conducted the fall semester; basically in the first couple of weeks of school
*2019 members simply need to fill out a form stating they plan to return if they want the ensemble they were in last year
*Audition information will be posted on social media pages (Instagram/Facebook), daily bulletin, and the Choir Website in the fall (probably early August before we technically start)

Marching Band

MTHS will have a marching band this fall. However, it is not clear at this time what this ensemble or its schedule will look like due to the governor’s current order.  As we move into June, please be looking for more information coming from our directors on how the marching band will operate this coming season.  If you are interested in joining Marching Band please email Mr. Hilligoss (zhilligoss@mths.us) or Mrs. Jarosz (kjarosz@mths.us)

STUCO - Student Council

Anyone interested in being on student council for the 20-21 school year, please send Mr. Finch an email with your name, school email address and phone number.  Any new freshmen can send Mr. Finch an email with their name and cell number. If you are going to be a senior or junior next school year and you have already been on student council for at least one year and you would like to be an officer on student council, please include that info with your email too. Any questions please email Mr. Finch at gfinch@mths.us

Tennis - Girls
Because of the ongoing construction on the MTHS tennis courts, frosh/soph girls tennis tryouts are going to be held from 6-8 pm next week, starting Monday, August 10 at the Washington High School tennis courts.  All of next week’s practices will be 6-8 pm at the Washington HS tennis courts. At Monday’s practice Coach Willard will give the girls additional information about the rest of the week.  We will just take the schedule a few days at a time at this stage.  Because of the restrictions on bus riding, players will provide their own transportation to and from the Washington courts. All girls need to be cleared by the MTHS athletic office to be allowed to practice or try out.  This includes a valid athletic physical and all required paperwork.
