Registering Your Student

If you are new to the Metamora area, you will want to register your child as soon as possible. The Main office is open during the summer months to accommodate you. During the school year, our Guidance Department will assist you.

For a smooth registration process, please provide the following documents:

Illinois State Board of Education Student Transfer Form (this is provided by the previous school)
Unofficial transcript from your former school and current schedule
Proof of Residency – photo ID with proof of District 122 residency – driver’s license or state ID, mortgage papers, property tax receipt, recent bill, etc.
Certified Birth Certificate
Immunization Records – proof of immunizations, including Tdap, for incoming Freshmen
Legal Guardianship Copies – if applicable

In addition, you will need to fill out a Demographics Sheet and various other forms, all of which are available via the link listed below.

HOMELESS STUDENTS (McKinney-Vento)

A student is considered “homeless” if the student is presently living in a shelter, sharing housing with relatives or others, in a motel/hotel or car, or otherwise lacking adequate housing. All homeless students have a right to immediate school enrollment, transportation services, and access to all academic and extracurricular activities offered to all students. If you have questions about whether a student qualifies as a homeless student or about the rights of a homeless student, please contact the MTHS Homeless Liaison, Dr. Jennifer Jewell, at jjewell@mths.us.  

IMPORTANT STATE OF ILLINOIS HEALTH REQUIREMENTS

The State of Illinois requires that all students entering 9th grade, or transferring in from out of state or out of the country, get a physical exam. A sports physical does not meet the criteria for the 9th grade physical.  Incoming Freshmen are also required to show proof of immunizations, including proof of a valid Tdap vaccination.

If you’d like MTHS to administer prescription medication, acetaminophen or ibuprofen to your child during the school day, please fill out the appropriate forms available via the Registration link listed below. Please refer to “Health Services” under the “Additional Facilities and Services” section of the Student Handbook Planner for more information.

Skyward Tutorial

A step-by-step tutorial on how to use our new online method of payment for fees and food service at Metamora High School is available with this link:  Food Service/Fee Management Skyward Tutorial.  Skyward allows you to pay registration and parking fees, as well as see that your child has money in his account to pay for lunches and breakfasts.  As an added convenience to parents, you can also track what your child is buying in the cafeteria.

Please be aware that every time you use your credit card there is a $1.50 convenience fee charged.
